What this website collects

This website collects personal information in exactly one place: the online prescription transfer form. If you fill it out, we receive your name, your phone number, the name of your current pharmacy, your delivery preference, and any notes you choose to add. That is all. The rest of the site is just pages to read.

Why we collect it

We use the information from the transfer form for one purpose only: to complete your prescription transfer and to contact you about it. We do not use it for marketing, and we do not add you to any mailing list.

Who can see it

Your form information goes into a restricted database that only pharmacy staff can read. We never sell your information, and we never share it with advertisers or other outside companies.

Cookies and tracking

This site does not set any analytics or advertising cookies. We are not tracking your visit.

How long we keep it

We keep your transfer request only as long as we need it to complete the transfer and keep a basic record of the request, and for any period the law requires. When it is no longer needed, it is deleted.

Your health records at the pharmacy

Once you are our patient, your prescriptions and health records are held at the pharmacy itself, not on this website. Those records are protected by Ontario's health privacy law, the Personal Health Information Protection Act (PHIPA), and Murray Shore Pharmacy is the health information custodian responsible for keeping them safe.

You are welcome to ask what information we have about you, ask us to correct it, or ask any question about this policy. Just call or email and ask for the pharmacist.

This policy is written and reviewed by the pharmacy. If our practices change, we will update this page and the date at the top.
